The Partnership for Healthier Nassau wants to know “what matters to you” when you think of health. The Community Health Improvement Plan survey is in both English and Spanish and is for all persons who live in Nassau County. Your ideas will shape our local Health Improvement Plan for 2019-2021. The survey takes 6-8 minutes to complete and the findings will be shared with the community in September at a strategic planning meeting. By completing this survey, you will help us create a new plan to support your personal health.
