Fernandina Beach, FL – The Florida Department of Health in Nassau County (FDOH – Nassau) is searching for past or present caregivers to complete the Healthy Brain Initiative (HBI) Road Map Needs Assessment Survey. The feedback will be used to identify resources and services that are needed within Nassau County for people with Alzheimer’s or related dementia and their caregivers.

The Healthy Brain Initiative will be important to Nassau County to lessen the future impacts of dementia by:

  • Providing support for caregivers
  • Advancing early detection
  • Use of evidence to inform policies, programs, and practices
  • Providing education and training to those who work in the public health and healthcare field as the population of people living with Alzheimer’s and related dementia grows

The survey is anonymous, unless the participant chooses to provide contact information. It takes an average of 8-10 minutes to complete.
