COVID-19 vaccines now available for 50 + years of age – Appointments begin Monday

The Governor’s EO #21-47 specifies that physicians can now complete FDOH Form (#DH8014-DCHP-03/2021, 64DER21-3) attesting to the medical necessity of CoViD Vaccine for their established patients.”

 

Executive Orders 21-47 and 21-62

The FL Governor has announced via Executive Orders that we are now allowed to vaccinate:
• Long-term care facility residents and staff;
• Persons 60 years of age and older;
• Health care personnel with direct patient contact;
• K-12 school employees 50 years of age and older;
• Sworn law enforcement officers 50 years of age and older; and
• Firefighters 50 years of age and older.

On Fri 19th March the Governor of Florida announced that starting Mon 22nd March those over 50 will also be eligible.

The Governor’s EO #21-47 specifies that physicians can now complete FDOH Form (#DH8014-DCHP-03/2021, 64DER21-3) attesting to the medical necessity of CoViD Vaccine for their established patients.

Those patients (or their caregivers) can then take the completed form, like an old-fashioned prescription, to a participating pharmacy for their vaccination, or the physician’s office may decide to send the completed form to the pharmacy directly via fax or some other electronic method.
