The Partnership for a Healthier Nassau invites the public to attend two separate Forces of Change community forums: May 15th, 3:00-4:00 pm at Amelia Island-Fernandina Beach-Yulee Chamber of Commerce, 961687 Gateway Blvd., Suite 101G, Fernandina Beach and in Callahan May 17th, 12:30-2:00 pm at the Pig Restaurant, 450102 FL-200, Callahan, Fl.

The Partnership for a Healthier Nassau facilitates Nassau County’s three-year Community Health Improvement Plan (CHIP). The CHIP is a countywide plan for community health systems partners and resource providers that aims to improve the health and wellbeing of its residents. The forum will brainstorm on trends, events and factors affecting the health and quality of life in Nassau County to create the Community Health Improvement Plan for 2019-2021.
