The Partnership for a Healthier Nassau invites all interested community members to attend the unveiling of the new 2016-2018 Nassau Community Health Improvement Plan (CHIP) at the Nassau Room, FSCJ Bean Center on Wednesday, February 24, 2016 from 2-4 pm.
Action Steps to address the three Strategic Health Issues of Access to Care, Behavioral Health/Substance Abuse and Transportation will be presented for input and feedback.
